If you have scheduled an appointment you will need to complete the Client Intake Form and Consent to Treatment prior to seeing Dr. Levy. When you sign the Consent to Treatment you are acknowledging that you have read the Notice of Policies and Practices document which is also contained on this page.

Printing and completing the Client Intake Form and Consent to Treatment before you arrive for your appointment will allow you to focus on your personal issues during your first visit, rather than using clinical time to complete paperwork. It is not necessary that you print the Notice of Policies and Practices, but please do read it.  It will help you understand your legal rights as a client, as well as my responsibilities as your clinician.

If you have medical insurance, please bring your insurance card with you to your first visit.
